vi.Supplier Comments or URL for Additional Information: This field is used if the supplier has any specific comments about the part in question, e.g. if the part will be replaced
soon or a URL where more information about the part can be found etc.
Note: If the contact and the Authorizer is the same person, it is not allowed to type
‘same’ in the fields. Instead, click on the Duplicate Contact Authorized
Representative button to duplicate the fields.
vii.Effective Date: The date on which this version of the part was made available
viii.Version: If the part is in the original version the word “Original” is put in this field
ix.Manufacturing Site: The D&B number of the site/sites where the part is manufactured can be written here
x.Weight: Item weight must be supplied in milligrams, grams or kilograms
xi.UOM: Unit of Measure, the drop down menu must be used. Make sure that the total weight on page 3 is consistent with declared weight on page 1

i.Terminal Plating / Grid Array Material: Pull‐down menu with 68 choices for the
plating on the terminals or material used as a termination for the electronic item(s).
ii.Terminal Base Alloy: Pull‐down menu with 8 choices. If there is no base material used by the component(s), such as those that only have plating for the termination,
select “N/A”
iii.J‐STD‐020 MSL Rating: Many of the plastic encapsulate are hygroscopic, The IPC/JEDEC joint standard describes the methods used to understand the moisture
absorption susceptibility of the package design. Use the pull‐down menu to select
the appropriate condition for the component(s) described on the form. There are 2
additional reporting conditions that may be used in addition: “Not Applicable”, used
to describe items that are not moisture sensitive, and “Dose not meet”, which must
be supplemented with the reason that this condition was chosen and the reason is
that it exceeds the requirements or it is less than the minimum of 24 hours.
iv.Peak Process Body Temperature: The peak temperature that the component can tolerate
v.Max Time and Peak Temperature: The maximum time the temperature of that component can tolerate
vi.Number of Reflow Cycles: Use the pull‐down menu to select the maximum number of cycles that the component or item can withstand (at the temperature and time
indicated in the previous 3 fields)

In the RoHS Material Composition Declaration Section, the following are required:
i.The Declaration Type: contains the legal language required by Ericsson already
appears in this section
ii.Select “Accepted” in the Supplier acceptance box indicating your acceptance of that statement
iii.Go to the RoHS Declaration field and use the pull‐down menu to make your selection regarding your material’s condition. Depending on the item selected, the
form will make adjustments to the required fields to follow. Your options include:
· Item(s) does not contain RoHS restricted substances per the definition above
· Item(s) contains RoHS restricted substances above the limits per the definition and is not under exemption
· Item(s) does not contain RoHS restricted substances per the definition above except for lead in solders and selected exemptions, if any
· Item(s) does not contain RoHS restricted substances per the definition above except for selected exemptions
· Item(s) is obsolete, no information is available
· Item(s) is unknown, no information is available
iv.In the RoHS exemption section, the following is required:
If RoHS materials are presented under an exemption, they must be selected from the
pull‐down menu Exemption List Version. These exemptions must also be claimed
next to the substances on page 3
v.The signing of the form must be done as the last step before returning to Ericsson, after all the substances on page 3 are filled in.

In the Homogeneous Material Composition Declaration for Electronic Products section:
Ericsson normally requires a full material declaration, meaning that constituents of any concentrations must be declared. Exempted are natural impurities meaning an undesired presence in very small concentrations which can be declared as a natural impurity. Ericsson is aware of the fact that some materials can be problematic to declare due to company secret. Please complete the form as follows:
i.Item/Subitem Name: the item for which a material declaration is requested on page 1
except when that item consists of several sub‐items. In that case the different sub‐items shall be named here. For instance transmitter, MUX, modulator, ect.
ii.Homogeneous Material: the specific homogeneous material on the electrode, resin, bonding
wire, ect.
NOTE: surface treatments and chemical coating are considered as homogeneous materials and must be declared.
iii.Weight: the weight of the homogeneous material. Note this should be the sum of individual substance weights within the homogeneous material. The sum of substance weights is not allowed to exceed the homogeneous material weight even by a small amount. Small differences due to scale inaccuracy are accepted but only between page 1 and 3
iv.Level: in the Level column the supplier should choose “Requester”, ”JIG A” or ”JIG B”
unless the substance to be reported does not exist in the pre‐defined lists, in which case “Supplier” should be chosen and the supplier must fill in the Substance category, Substance and CAS number.
NOTE: All substances must be declared!
If an incorrect CAS number or no CAS is given, the declaration will BE
DISQUALIFIED
v.Substance Category: to be chosen from the pull‐down menu if Level is “JIG A”, “JIG B” or filled in if Level is”Supplier”
vi.Substance: If one of the prefabricated lists was chosen, choose the substances from the pull‐down menu. If a substance needs to be Supplier defined, enter the substance here.

vii.CSA: if a CAS number exists the supplier must fill in it if the “Supplier ” Level is chosen. It
is extremely important that the correct CAS# is given
viii.Exempt: if a substance is exempted in accordance to RoHS it should be stated with the appropriate exemption number in the column Exempt.
ix.Weight: is the weight of the substance in the homogeneous material, must be declared together with the correct Unit of Measurement(UOM)
x.Unit of Measurement: stated in g or mg
xi.PPM: if applicable enter a value
