正常胎儿脑沟超声观察及形态分析

【摘要】目的观察正常胎儿大脑外侧裂(S F)、顶枕沟(P O F)及距状沟(C F)等脑沟的形态发育特点,以及数据的测量,掌握胎儿脑沟回发育的特点。方法收集18〜36周的单胎胎儿,记录SF、POF以及CF出现时间,形态特点,并测量SF高度,上缘宽度以及额叶颞叶间未覆盖的最小距离;POF和CF宽度、高度以及顶端角度结果胎儿孕18周能探及SF,为平滑弧形,在19周后其上缘变平,21周SF几乎均变成梯形。此后,上缘长度随孕周缓慢增加;24周开始上缘两侧逐渐向脑实质内伸展,27周几乎所有胎儿均可见这一表现,此后上缘继续延伸,两脑叶间 的未覆盖区逐渐缩小。POF在19-20周左右出现,最晚21周可被探及,在整个孕期宽度前期缓慢上升,后期缓慢下降,23周左右达到最宽,整个 孕周高度逐渐增加,失角逐渐变小。CF的出现较POF略晚,约在20-23周出现,最晚24周可被探及CF。其形态的变化过程与POF类似:结论SF 18周已出现,21周均由弧形转为梯形,25周左右成矩形,27周成“官帽”形。POF 21周最晚出现,26周左右时宽度与高度大致相等。CF最晚24 周可被探及,在27〜28周时宽度与高度大致相等。各大脑沟深度与孕周增加呈正相关,两脑叶间未覆盖区和顶枕沟角度与孕周增加
呈负相关。【关键词】大脑外侧裂顶枕沟距状沟
parieto—occipital fissure ( POF ),and calcarine fissure ( CF ) in normal fetus in order to master the characteristics of fetal sulcus gyrus development. Methods The single fetuses from 18 to 36 weeks were collected, the appearance time and morphological characteristics of SF, PO F, and CF were recorded, and the height of SF, the width of the upper edge, the minimum uncovered distance between the frontal and temporal lobe, the width, height and top angle of POF and CF were measured. Results The fetus detected SF in pregnancy 18 weeks, but at this time it was a smooth arc. After 19 weeks, the shape of SF gradually changed, and its upper edge flattened. At 21 weeks, the fetus's SF almost becomes trapezoid. Thereafter, the length of the upper edge slowly increased with the gestational week; from the 24th week, both sides of the upper edge gradually extended into the brain parenchyma, and this phenomenon was almost visible to the fetus at 27 weeks. Thereafter, the upper edge continued to gradually extend, and the uncovered area between two brain lobes gradually shrinks. POF appeared around 19~20 weeks, and was detected at the latest 21 weeks. The width of the entire pregnancy period slowly rised in the early stage, and then decreases slowly in the later period, reached the widest around 23 weeks. The height gradually increased throughout the pregnancy week, and the angle gradually decreased. The appearance of CF was slightly later th
an that of POF. It occured about 20~23 weeks, and CF was detected at the latest 24 weeks. Its morphological change process was similar to that of POF. Conclusion SF appears at 18 weeks, turns from arc to trapezoid at 21 weeks, becomes rectangular at about 25 weeks, and become "official hat" at 27 weeks. POF appears the latest at 21 weeks, and its width «ind height are approximately equal at 26 weeks. CF can be detected at the latest 24 weeks, and its width and height are approximately equal at 27〜28 weeks. The depth of each sulcus is positively correlated with the increase of gestational age, while the uncovered area between the two lobes and the angle of parietooccipital sulcus are negatively correlated with the increase of gestational age.

脑沟冋的发育异常是一组预后极差的先天性异常,  1 资料与方法
主要包括:巨脑回畸形,无脑回畸形和多小脑回畸形等。轻症患者可有不同程度的学习障碍、癫痫以及注意力损害,严重者表现为智力迟钝,语言发育和社交功能严重受损以及视力和听力丧失,甚至不能存活。有研究表明脑沟回的发育异常可作为筛查小头畸形、脑沟回发育迟缓、半侧巨脑畸形等疾病的客观依据m。也有学者认为当脑沟深度测值小于同孕周正常值三个标准差时,则 脑沟可能发育不良[2\自胎儿M R I应用于临床后,有部分脑沟回异常的胎儿在产前明确诊断i3-5],超声检查可以观察到胎
儿脑沟回结构,且随着孕周的发展形态出现变化t w l,而这一变化为超声筛查脑沟回异常提供可能本文分析正常胎儿脑沟回的形态发育特点。1.1临床资料选择2018年9月至2019年9月杭州市妇产科医院18~36周单胎胎儿。首先确定胎儿的孕周,根据孕妇末次月经及早孕期超声检查结果,推算出胎儿准确孕龄,然后行常规产科检查,测量双顶径、头围、腹 围及股长,计算胎儿估计孕龄,胎儿真实孕龄与检查当时的估计孕龄差距在1周内的,常规超声检查无异常,孕 母早、中孕筛查为低风险,孕母无妊高症、妊娠期糖尿病等并发症纳入组内。以月经龄或纠正孕龄为标准,每孕周为1组,每组20个胎儿,孕妇年龄18-48岁。本项目经医院伦理委员会批准,所有受试者均知情同意。
1.2方法采用美国G E-V o l u s〇n E8、E10彩多普勒超声诊断仪。进行常规产科检查后,观察胎儿大脑外侧裂(S F)、顶枕沟(P0F )和距状沟(C F )。S F观察方法:先获得标准双顶径平面,然后探头略向尾侧移动, 显示清晰的大脑外侧裂,大脑外侧裂在侧脑室的外侧与颅骨光环之间,将大脑的额叶颞叶与顶叶分开(见图

1),测量其高度,上缘宽度及额叶颞叶间未覆盖的最小 距离,同时观察上缘有无向两侧额叶及颞叶延伸。然 后观察PO F:将探头转至侧脑室平面,轻微调整探头,在侧脑室后角与脑中线间可探及三角形的结构,其顶端 对着侧脑室后角(见图2),测量其宽度高度以及顶端 的角度。最后观察C F:获得经小脑斜切面,将探头转 90° ,显示小脑幕结构,在小脑幕上可见两个三角形的 脑沟结构,分别是POF和CF,位于上方的是POF,下 方的是CF (见图3 ),同样测量C F的宽度高度及顶端 的角度。所有图像均先留存,检查结束后记录其形态特 征以及测量的相关数据,测量3次取平均值。为了保证 测量数据的准确性,随机选择20例胎儿,使用双肓法 由2位医师分别测量SF、POF及CF相关数据,均测量 3次取平均值。
1.3统计学方法采用SPSS21.0统计软件。计量资料以 U± s)表示。组内一致性评估采用组内相关系数(ICC)。

2结果
2.1S F的各结构特征以及POF、CF在各孕周的显示 率SF、POF和CF在孕期随着孕周的增大而出现,且 其形态出现改变。SF早期仅为一略向脑中线隆起的弧 形结构,然后发育成为一类似梯形的“城墙样”结构,以后,大脑外侧裂上缘的两端会进一步的向额部或颞部延伸,形成一“官帽样”结构。见表1。

脑沟回是指大脑的表面凹凸不平,凸起的部分称 为脑回,凹下的部分是脑沟。在胎儿的发育约2个月时,大脑的表面出现脑沟回轮廓;6个月时,脑沟回出现凹 凸,大脑皮层层次结构也基本定形。8个月,大脑皮层 更为发达,表面主要沟回已经完全形成,虽然人体中枢 神经系统直到儿童时期才完全成熟,但可以通过观察胎 儿脑沟回的变化提前评估了解胎儿大脑发育情况[1(1]。
超声检查可以观察到胎儿的较多脑沟回结构,其 表现为脑实质表面或内部强回声,随着胎儿脑部的发 育,可观察到这些脑沟回从平滑的弧线到局部形态出 现变化,直至形成较多褶皱深人脑实质的过程,而这 一形态变化过程为超声筛查脑沟回异常提供可能。超 声检查可以通过观察及测量各类脑沟回的形态变化,筛查出可能存在脑沟回异常的胎儿,尤其在侧脑室增 宽[||]、宫内生长迟缓[|2]及心脏异常的胎儿[|3]。由MRI进一步诊断,使脑沟回异常的胎儿在产前被诊断. 减轻日后患儿家庭和社会负担,最大程度上降低筛查 畸形儿的成本。
本资料结果显示,在孕18周胎儿能探及SF,但此 时为一平滑弧形,19周后S F的形态发生改变,其上缘 变平,21周胎儿的SF几乎均变成梯形。此后,上缘长
