液压软管接头
2013年2月12日，在某平台上进行吊车回流软管更换的过程中，发生了一起软管接头打伤作业人员牙齿的事故。

1、 事故描述

事故发生时，平台正在进行吊车大小钩液压回流软管的更换工作。旧的软管拆除后，维修人
员开始进行新软管的安装工作。 鉴于现场作业空间非常有限，作业人员使用牵引绳（麻绳）拉拽软管接头以便使得软管接头（A）就位。在拉管线（使得接头就位）的过程中，软管的另一端（B）意外弹出，其金属接头部分击中了作业人员牙齿，造成三颗牙齿不同程度损坏。
When the accident happened ,the offshore platform is to replace hydraulic return hose of crane hook. After the demolition of the old hose, maintenance personnel to new hose for installation. Because the working space is very limited,operators use the tow rope (hemp rope) pull hose connection makes the hose connector (A) in place. In the process of pulling the 人造板生产线hose, the other end of the hose (B) the pop-up unexpectedly. Metal hose connector part hit operator’s teeth, three teeth appear different degree of damage.
2、 事故原因分析
The cause of the accident analysis
1） 作业安全分析（JSA）不详细，没有识别出危险，没有防护措施。
1) The job safety analysis (JSA) is not detailed, did not identify the danger, no protective measures.
2）缺乏风险防控意识：拉拽软管接头（A）有可能造成接头（B）端弹出打伤人员。
Operators lack of consciousness of risk prevention and control: Pull hose connector (A) is likely to cause joint（B）end pop-up wounded operator.
3）设备或材料未能固定：因为作业中没有对软管接头B端固定，导致其接头意外滑脱弹出，击中作业人员牙齿。
Equipment or materials have no fixed: because the job without fixed for hose connection B end, lead to accidental slip pop-up, the joint hits the operator's teeth.
3、防范措施：
Safety Protection Measures
1）提高人员安全意识。
Improve the personnel’s safety awareness.
2）工作安全分析（JSA）必须要细致、有针对性，并做好防护措施。
Job safety analysis (JSA) must be carried out in detail with explicit targets and comprehensive protective measures
3）强化设备修理过程中的安全监护工作。
Strengthen the safety supervision in the process of equipment repairment.
